In the basement of the health resort west wing. Opening it needs the Knossos LLC facility key.
Access the underground facility
The Labyrinth
Entry is by transit from Shoreline only, and each run consumes a Labrys access keycard. Insurance does not work here: anything left behind is gone for good.
Clear your spawn chamber
The Labyrinth
All five spawn chambers are sealed behind a hazard you have to solve or die to. Chamber 1 is a toxic pool (Key 01, then the wall button). Chamber 2 is steam (Key 02, then the valve handwheel from the second floor applied to the pipe by the yellow door). Chamber 3 is fire (Key 03, then a BBQ-S43 gas torch on the first door, then the electrical cabinet button). Chamber 4 is a shotgun trap (Key 04, crawl through, pull the lever left of the door). Chamber 5 is a toxic puddle (gas torch on the round door, then the wall lever).

Five notes, one per body: Assistant's notes on Group #3 under the right hand, on Group #8 under the left arm, on Group #3 and Group #10 beside the body, on Group #12 under the body, and on TG-Vi-24 test subjects under the head. The fourth needs the Observation room key.

The locked office is the observation room, and it needs the Observation room key.
Listen to the audio tape from the office

The Deceased scientist's audio tape is on the ground under the scientist's right hand, and has to be listened to.
Hand over the audio tape to Jaeger
Pays 500,000 roubles plus an Accuracy International AXMC .338 LM, two magazines and two ammo packs.

Shoreline
Not in the Labyrinth at all. The report is back on Shoreline, at the drain pipe next to the pier. Reading it unlocks the Theseus achievement.
